- Service Learning Project Participation. Student must participate in a minimum of one service learning project (4 hrs) during the current school year. Teacher will verify service learning project completion with a signature on the Student Portfolio Tracking Sheet and student will include the completed Service Learning Documentation form in their student portfolio. Giving back to your community can boost your scholarship resume. If you are looking for local volunteer opportunities, Utah Volunteer Center Association and YouthLinc have opportunities waiting for you!

- Attend leadership/service conference. Student must attend at least on leadership conference during the academic year. Verification will be indicated with their teacher’s signature and a copy of their leadership conference participation certificate in their student portfolio.
- Peer tutoring/mentorship. Student must participate in the peer tutoring or peer mentoring program. Teacher will verify participation in peer tutoring/peer mentoring program with a signature on the Student Portfolio Tracking Sheet.
- Student government participation. Student must participate student government. Teacher will verify participation in student government with a signature on the Student Portfolio Tracking Sheet.
- Making a Difference-MAD JAGS (4+ hours). Student must complete at least 4 hours of MAD JAGS service. Teacher overseeing MAD JAGS will verify participation in MAD JAGS with a signature on the Student Portfolio Tracking Sheet and the student will include the completed MAD JAGS Participation Documentation form in their student portfolio.

- Other (approved by counselor ahead of time). Should there be extenuating circumstances that prevent a student from being able to complete one of the supplementary items in the Leadership portfolio component, a student may apply for a substitution using an "other" supplemental activity as outlined on the "Other" Supplemental Activity Documentation form listed below.
